Life Transition Support
Supporting neurodivergent individuals
through life’s big (and small) changes
Times of transition can be especially challenging for neurodivergent individuals.
Transitions are a normal part of life — but for neurodivergent individuals, they can be especially overwhelming.
Whether it’s a new school, a shift in term routine, the move into adulthood, or the leap into work or further study, these changes often disrupt the predictability that many neurodivergent people rely on. Once a routine has been established for a particular life stage, leaving it behind can cause anxiety, dysregulation, and emotional exhaustion.
At Social Living Solutions, we understand that transitions aren’t just logistical — they’re emotional, sensory, and social. Our Life Transition Supports are designed to walk alongside you or your child through key points of change with neuroaffirming, strengths-based support.

Types Of Transitions We Support
school years
Transitions betweenEven moving from one year to the next in the same school can bring new teachers, classrooms, peers, and expectations. We prepare children and teens for these shifts by building emotional readiness, introducing practical strategies, and supporting self-advocacy.
Primary to High School
Transition fromThis is one of the biggest shifts in routine and responsibility. We help students prepare for managing timetables, moving between classrooms, using lockers, navigating larger campuses, and building new social connections.
High School to College or TAFE
Transition from Young people may need support developing independent learning skills, time management, and understanding how to access learning support or reasonable adjustments.
Transition from College to University or Work
This leap requires a new level of self-direction. We guide young adults through setting up daily routines, managing executive functioning demands, navigating public transport, and preparing for workplace or university life.
Transition Between School Terms and Holidays
Routine changes during school holidays can significantly impact emotional regulation. We help families prepare for these changes with strategies to manage downtime, maintain structure, and ease the return to school routines.
Returning to Education or Work After Time Away
For some neurodivergent individuals, life may include a break from study or employment. We offer supportive re-entry planning, helping rebuild confidence, re-establish routines, and address sensory or emotional needs
Life Stage Transitions
We also support other transitions such as moving out of home, starting independent living, or navigating shifts in support systems (e.g. from child to adult services). These moments can be complex, and we’re here to help build self-determination and clarity.
